Evaluation

  • In outpatient occupational therapy, after receiving an Occupational Therapy referral from a physician and completing a chart review, the occupational therapist on the patient’s care team conducts an evaluation to determine the patient’s current functional status, specific impairments, and occupational performance challenges.

  • This evaluation assesses physical, cognitive, emotional, and environmental factors impacting the patient’s ability to engage in meaningful daily activities.

  • The findings guide the development of a personalized intervention plan focused on optimizing independence, improving functional abilities, and achieving the patient’s occupational goals.
